Geriatric Care Consultants is a professional team of dedicated Geriatric Care Managers who perform all care management functions. We service Westchester County in New York and Lower Fairfield County in Connecticut.
Our staff works as a team to find the best solutions for you, your aging loved ones, and your family. Geriatric Care Consultants is a proud member of the Aging Life Care® Association. All of our care managers are Advanced Professional Members of ALCA®

Karen J. Schwartz
Honorary Founder
Karen J. Schwartz, MSW, LCSW, ACSW, C-ASWCM has had over 30 years of experience in directing clinical services, consultation, supervision, and administration in long-term care facilities, adult day health care programs, and geriatric care management. Karen was also an adjunct professor at the College of New Rochelle, School of New Resources. She is a New York State Licensed Clinical Social Worker and Certified Advanced Social Work Case Manager / Aging Life Care Professional. Karen holds a Master of Social Work Degree from New York University in Manhattan, NY.

Anne Bergman
Geriatric Care Manager
Anne Bergman RN, BSN, CCM, obtained her BS in Nursing from Fairfield University in 1981. The majority of her direct patient care work experience was in the Coronary Care Unit and the Cardiac Catheterization Lab at New York Presbyterian - Weill Cornell Medical Center. As the first nurse to work in the Cardiac Catheterization Lab, she was instrumental in setting up the quality assurance and patient transfer programs. She then worked with the Chair and Vice-Chair of the Department of Medicine at Weill Cornell Medical College as the Length of Stay Nurse to identify and remediate the reasons for increased length of stay in the patient population. Anne was the director of the Patrick Durkan Fencing Foundation, a non-profit supporting Olympic level fencers. Anne is thrilled to be part of the Geriatric Care Consultants team where she is utilizing her nursing skills in her role as a geriatric care manager.

Amy Lazar
Geriatric Care Manager
Amy Lazar, MSW, LMSW graduated from Boston University with a Bachelor’s degree in Psychology, and with a Master’s degree in Clinical Social Work from Fordham University. Over the last 20 years, Amy has acquired a wide range of social work experience including extensive work in Foster Care and Adoption with Louise Wise Services, School based mental health with Children’s Aid Society, and Patient Care at Hospital for Special Surgery and Mary Manning Walsh Nursing home. Most recently, Amy had provided Geriatric Case Management with Westchester Jewish Community Services, for Seniors receiving Home Delivered Meals and various services throughout Westchester County.
Amy is a dedicated social worker with a passion for contributing to the welfare of others. Her professional goals and successes derive from compassion, teamwork and professionalism- always with the client’s best interest in mind. Amy has been raising her family of three children in Westchester for the last 16 years. She had the pleasure of serving on the PTA and was Committee chair of the Elementary After School Program for several years.
Amy Lazar is very enthusiastic to be a team member at Geriatric Care Consultants. It is with pride, reliability, respect, and dedication that she looks forward to working with aging clients, their families. Amy looks forward to the opportunity to support the unique, individual needs of the client and family, which is an essential role of the Geriatric Care Manager.

Allyson P. Mann
Geriatric Care Manager
Allyson P. Mann, MSW, LMSW, C-ASWCM, comes to Geriatric Care Consultants with extensive experience in the field which started with the close relationship she had with her grandmother. At the age of 15, Allyson started volunteering at a local nursing home. She later went on to become a Recreation Therapist, and then pursued a MSW from Fordham University. Allyson has experience as Director of Social Services at several nursing homes, working in long-term care and short-term rehabilitation. She acts as a private consultant helping prepare nursing homes for their State Department of Health annual survey. After 22 years working in the nursing home industry, Allyson joined Geriatric Care Consultants in August of 2015 and has since obtained an LMSW and a C-ASWCM.

Beth Metzendorf
Psychotherapist
Beth Metzendorf, MSW, LCSW, Psychotherapist graduated from The University of Michigan with a bachelor’s degree in Psychology and New York University with master’s degree in Social Work. With more than 20 years of clinical social work experience, Beth practices a tailored, individualized, flexible style, with an appreciation for the many challenges associated with aging to best support clients.
Beth approaches each client relationship with a deep respect and curiosity for each client’s unique history. By creating a safe space in which to feel understood and supported, clients are able to formulate their own best choices.

Dyanne (Dee Dee) Thompson
Office Manager
Dyanne (Dee Dee) Thompson, with expertise in administration and operations, brings a variety of skill sets as an office manager. Dee Dee spent six and a half years as the Production Coordinator for the custom framing department at A.I. Friedman in Port Chester. Prior to that she was the Administrator for the Westchester-Putnam School Boards Association for eleven years. She received her Bachelor of Science degree from Skidmore College.
Included amongst Dee Dee’s community service are eight years as a PTA president in the Mamaroneck School District, co-chair of the Mamaroneck UFSD’s Centennial Celebration and a Committee co-chair for the United States Open, in addition to service on many boards. Dee Dee is delighted to be working in the community she loves and having the opportunity to help make people’s lives a little easier.
